And, although, things may have felt uncomfortable at first Alejandro, I do see where Dani has expressed concerns. Thank you (Discussion 5.1, 2022)..Me: Alejandro, with Mexican traditions being so rich, I understand that Dia de Las Madres, the second most important day in the year, is actually today, May 10th, where all of your family will be coming together to celebrate your mothers and important women in your lives, like your wife (King 5, 2018). Celebrating family is very important, and I respect the close extended Catholic family that you and your wife, Dani, have. Your safety and the safety of your family is the utmost importance to me. I will be completing the appropriate paperwork at this time, based off current events and information received from your wife to have you committed to an involuntary stay at our psychiatric inpatient hospital. Please know I weighed all possible outcomes and found this to be the safest for all involved due to your unwillingness to share your intent with your unsecured guns in the home (Discussion 5.1, 2022).Alejandro: I don’t understand. If you know all of my relatives are coming to my home to celebrate Dia de Las Madres why would you even consider this?! They will ask questions of where I am. You’ll disgrace me and my family name. I am not a threat to myself or anyone else. Me: With your wife’s statement when you both arrived of concern where you have purchased a .45 and having it accessible, having your shotguns accessible, where you have been spending money left and right when you and your wife cannot afford it, and your notable anger and agitation when Dani attempts to discuss these concerns are enough to tell me that I have improperly diagnosed you. Alejandro, I apologize. I did make a mistake. These signs and symptoms do tell me that your proper diagnosis is Bipolar Disorder, and I would like you to go inpatient to become stabilized (Discussion 5.1, 2022). Alejandro: (Becoming agitated…) Look, I have tried to tell Dani we’ll be okay. I am looking for another job. I try to protest to let her know I am just trying to protect our family. And, like I said, I plan on having another job real soon (Discussion 5.1, 2022). Me: (Once law enforcement is on scene and ready….) Alejandro, no disrespect, I see that you are becoming agitated, and that is not my intent. I have already completed the paperwork for an involuntary inpatient stay (Missouri’s affidavit…, 2000). Our local law enforcement per the State of Oklahoma will transport you. And, per their policy, not because you are under arrest, the State of Oklahoma requires that all emergency detained individuals be placed in handcuffs for transport. You will be taken to the inpatient hospital at Fort Supply which is just shy of two hours away. It was the first location with an available male bed (Pfau, 2022). *At this time, I would stand up and open the side door to allow law enforcement to come in and take Alejandro into protective custody/emergency detention and transport him per their guidelines. I would step out and inform Dani of the situation and offer for her to see him once more if desired and to encourage her to take a bag of clothes, personal belongings for Alejandro.State of Oklahoma Laws and Rules about Involuntary CommitmentThe state of Oklahoma requires a Licensed Mental Health Practitioner (LMHP) evaluation be completed for voluntary or involuntary commitment. Medical clearance is required prior to going inpatient to a mental health facility. The process may be found at 43A O.S. §§5-410 et seq which follows district court proceedings to determine if the individual is a person requiring mental
health inpatient treatment and that the least restrictive and most appropriate treatment required (Wylie, 2020).Of course, the individual must have caused fear for their own or other’s safety /lifewhere law enforcement is required to be on scene. Once this is determined ONLY via a Licensed Mental Health Provider (LMHP) evaluation and statement (which may now also be obtained on scene via telehealth by a trained peace officer), the individualt will be taken into protective custody and detained until a location for placement is confirmed. This involuntary commitment mus also be approved by a judge and may be upheld for up to 120 hour or five days (Pfau, 2022).Evaluations by an LMHP may occur on scene where law enforcement was summoned as stated earlier, and other times they may be completed in the emergency room or in a clinic or an LMHP’s office during an intake, med check, post-hospital stay, etc. The individual must be evaluated within 12 hours by a LMHP. IF placement to an inpatient stay does not occur swiftly, a new LMHP evaluation may be required prior to inpatient acceptance (Wylie, 2020).

It’s the day before a family holiday (such as Thanksgiving, Easter, Dia de Muertos, or the like; choose a specific holiday to use), and relatives are coming to the house. You know that Alejandro and Dani have a close extended Catholic family.
Hello Alejandro, I understand that you have a thanksgiving party at your house. However, having a family holiday will not make me release you from the hospital since I feel that you are not safe. To ensure that you are safe and stable, I will admit you. One of the reasons that have made me consider admitting you is that the prescribed medications have failed to work as expected. This is evidenced by the fact that your depressive symptoms have worsened (Shea, 2016).
I am aware that you possess a gun which compromises your safety and that of family members and other people in the community. I feel that your safety is guaranteed in the healthcare facility compared to when you are at your house. Going home is risky for the family members and relatives. Since your depression has failed to respond to the prescribed medications, it is important to hospitalize you since you are likely to develop homicidal and suicidal ideations.
Admitting you to the hospital is important since we will have an opportunity to thoroughly investigate your condition and make an accurate diagnosis. We will also be in a position to initiate you on new medications that will relieve your depressive symptoms and improve your mental health and quality of life. We will also help you reduce extravagance and spending sprees by initiating behavioural therapy sessions. Being in the hospital has multiple benefits to you and to your family members in comparison to being at home. Generally, we will implement various interventions which will lead to the stabilization of your overall functioning (Sadock et al., 2014).
